If you need nursing home care, your primary care physician may recommend that a Great Lakes Geriatrics provider manage your care during your extended stay. Here are some things you should know about our team:

Great Lakes Geriatrics Physicians

have their entire practice in the extended care facility, are entirely focused on extended care and have had special training in care for acute and chronically ill patients.

Your Primary Care Doctor will still be in charge.

The Physician will be your doctor only for your extended care facility stay. They will communicate with your Primary Care Phyisician and work with them and other specialists, as part of a team, to manage your care.

Our providers will know you and your history.

Your Physician will be briefed by your doctor and will study your records before you arrive. On your first day, they will meet with you and your family to get aquainted, talk about your care plan, and answer your questions.

Your Geriatric Specialist will always be within reach.

We are on call 24 hours a day, 7 days a week. Because we practice only in the extended care facility, we can be reached quickly when you a question or need special attention.

Your Physician will be a good communicator.

It is important and comforting to hear clear explanations and have questions answered promptly. Being a good communicator is part of being a good physician.
